When you contact us or use a care-guidance form, you may provide your contact details, preferred area, communication preferences, budget information and descriptions of support needs. Professional and community contacts may provide referral and service information. Some secure workflows request additional details only when necessary.
How information supports your care search
Information provided to Find My Care may be used to respond to inquiries, understand care preferences, assess possible provider options, coordinate authorized referrals, communicate with you, maintain service records and meet applicable legal obligations. A public listing is not a verified recommendation.
Sharing and service providers
Referral information may be shared with communities and healthcare professionals as necessary for the services you request and subject to the applicable disclosure and consent process. Hosting, email, text and other technology providers may process information to help operate the service. Ask us how a particular referral will be handled before authorizing it.
Sensitive care information
Names, contact details, care descriptions and other contextual information can be sensitive when connected to an identifiable person. Do not send medical records, identification documents, Social Security numbers or other highly sensitive files through a general inquiry, ordinary email or text message. Ask Find My Care or the receiving provider for the approved secure method.
Website analytics
Public informational pages may use Google Analytics to understand aggregate page visits and improve the website. Analytics is not enabled by our website code on the staff workspace, signing links or disclosure-completion pages. Browser settings or extensions may restrict analytics.
Questions, corrections and ending referral services
You may contact us to ask how your information is used, request correction of information, or send a written or electronic notice ending referral services and use of personal information under applicable Arizona referral law.
