Preparing for a Senior Care Assessment

Prepare clear everyday examples and questions for a community care assessment, including care routines, preferences and costs.

Bring everyday examples

Instead of trying to choose a care label yourself, describe what happens during a typical day. Explain which tasks need reminders, hands-on help or supervision, and note recent changes. Include what helps the person feel comfortable and understood.

Include the people who know the routines

A family member, current caregiver or involved professional may each know different details. Ask the receiving provider which information it needs and how to send it securely. Do not put sensitive records into a general website inquiry.

Ask how the assessment affects the offer

Ask whether the provider can meet the identified needs, which services are included, which cost extra and whether a room or move-in date has actually been confirmed. Request the resulting care and pricing information in writing.

Plan for reassessment

Ask when needs are reviewed again, how changes are communicated and how fees may change. Keep a contact person and a list of outstanding questions so the transition does not depend on assumptions.
